Survivorship Clinic
The Survivorship Clinic is available to Concord Hospital oncology and haematology patients who have completed chemotherapy for an early stage cancer and who have no sign of a recurrence of their cancer.
The first visit takes around two hours. During this visit a survivorship care plan is developed. You will be seen by the following professionals:
- Specialist cancer doctor (Medical Oncologist or Haematologist)
- Cancer care nurse
- Dietician
- Exercise physiologist
- Clinical psychologist
Survivorship care plan
Your care plan will include a summary of the type of cancer you have had, the treatment, what follow up you will be having, who will be doing the follow up for you and things to look out for.
Each member of the multidisciplinary team who you see in the clinic will make recommendations to help you in achieving the best health possible. A copy of your plan will be sent to you, your GP and each of the cancer doctors involved in your care.
Survivorship Clinics are held weekly. Clinics for new patients are held on a Thursday afternoon. Oncology follow-up patients are seen on a Monday morning with every alternate Clinic visit completed via phone on a Thursday morning.
Haematology survivors return to their original medical team for ongoing care.
Oncology survivors either return to their original medical team or continue in the survivorship follow-up clinic. In the follow up clinic, most people are seen every three months by a medical oncologist and a survivorship nurse specialist.
