GP CanShare – CancerPlus

GP CanShare is a team of experienced cancer and palliative care workers who support cancer patients through their program CancerPlus.

CancerPlus is our free education support service for patients with cancer.

All cancer patients (and their carers) undergoing treatment or receiving care at Concord or RPA Hospital or Chris O'Brien Lifehouse are eligible to access this service.

We focus on helping you live well after a cancer diagnosis. Our goal is to make sure you have the support and help you need during this difficult time. The CancerPlus program is provided by our GP CanShare team of experienced care coordinators.

During a CancerPlus session our team will work with you to find out what kind of support you need, connect you with support groups specific to your cancer and treatment and provide resources and referrals.

How does CancerPlus work?

When you come to see us, we will chat to you about what your health goals may be, and what we can do to help you continue to do the things you enjoy. We will also ask who your regular GP is, and help you find a GP if you don’t have one or want to change GPs. You can also choose to have your GP linked with us to provide them with updates. You get to decide what information is sent to your GP, if any, and we will abide by your wishes.

What if I need to change my appointment?

If you need to cancel or reschedule for any reason, please let us know as soon as possible. If you are running late (by more than 15 minutes) please let us know so we can wait for you.
