Accessing Information held by Sydney Local Health District
Under the Government Information (Public Access) Act 2009 (GIPA Act) there are four ways in which information held by Sydney Local Health District is made available:

Sydney Local Health District website (slhd.health.nsw.gov.au): You can search the website for information about the District, its facilities and services, recruitment and training, as well as a list of the District’s publications, resources and policy documents.
Agency Information Guide
Disclosure Log: The Disclosure Log is a record containing details of government information that has been formally released under the GIPA Act that the District recognises may also be of interest to other members of the public. Only non-personal information is included in the Disclosure Log, with the information provided being what was available at the time of the request.

Information tabled in NSW Parliament: No documents have been tabled in NSW Parliament by, or on behalf of, Sydney Local Health District since the commencement of the GIPA Act. When information concerning Sydney Local Health District is tabled in parliament, it will be available on this page.
Information not disclosed: There is no open access information not released by Sydney Local Health District due to an overriding public interest against disclosure.

Annual report

Policy documents: Sydney Local Health District is a statutory health corporation under the Health Services Act 1997 that is guided by the following policy documents:
- NSW Ministry of Health Policies; and
- Sydney Local Health District policies
NSW Ministry of Health policies can be accessed via the Policy Distribution System.
Sydney Local Health District develops local policies where no equivalent NSW Health Policy Directive exists. The District’s policies are developed primarily as District-level policies, with stand-alone Facility or Service level documents developed by exception only.

Sydney Local Health District's policies are constantly under review and are currently being updated. It is anticipated that additional policies will become available in due course.
District policies can be accessed by contacting the District’s Policy Manager at SLHD-Policy@health.nsw.gov.au, or the SLHD Right to Information Coordinator at SLHD-GIPA@health.nsw.gov.au.
Sydney Local Health District regularly reviews its authorised proactive release program. Any information that is released can be found on the District's website.
If the information you are seeking has not already been published by Sydney Local Health District, but is information which raises no particular concerns in terms of possible public interest reasons why it should be kept confidential, then Sydney Local Health District may be able to release the information to you without the need for a formal application.
If you think this may apply to the information you are seeking you can email the Right to Information Coordinator at SLHD-GIPA@health.nsw.gov.au to make an informal request.
Under the GIPA Act, however, NSW Government agencies are not required to release information without a formal application. Sydney Local Health District reserves the right to require you to lodge a formal application, particularly if there may be significant public interest considerations that need to be taken into account in deciding whether the information can be released or if you request a large volume of information or if it would otherwise take the District a significant amount of time to consider your request.
If the information you are seeking is not available on this website and is not otherwise routinely provided by Sydney Local Health District on request, then you have a right to formally apply for access to specific information.

A $30 application fee is payable via cheque/money order, or by credit card using our online payment portal.

As stated in the Act, we are only able to accept access applications that:
- are in writing and sent to us by one of the methods above;
- clearly state you are requesting information under the Government Information (Public Access) Act 2009 (NSW);
- enclose the $30 application fee;
- have a return address as the address for correspondence; and
- include as much specific information as necessary to enable us to identify the information you are seeking.
If your application does not include these five things, it will be invalid and will not be processed. If that happens, however, we will let you know and we will help you, if possible, by explaining how you can make a valid application.
Otherwise, we will write to acknowledge receipt of a valid formal application within five working days and will consider your application within 20 working days (subject to any extension allowed under the Act). If any extension of time is required to consider your application, we will let you know in writing.
Review of decisions
If you have formally applied for information held by Sydney Local Health District under the GIPA Act and you are dissatisfied with the outcome of your application, you may be able to:
- seek an internal review through Sydney Local Health District
- approach the Information and Privacy Commission NSW for an external review of the outcome
- request a review through the NSW Civil and Administrative Tribunal
You will be given details about your review rights when the outcome of your application is provided to you.
Internal Review
An internal review is conducted by someone who is not less senior than the person who made the decision and who was not involved in making the original decision. The reviewer will make a new decision as if the original decision had never been made.
You can apply for an internal review within 20 working days of a decision (or, if we don't decide your application in time, within 20 working days of the date it should have been decided which is the date of deemed refusal).
To apply, complete the Sydney Local Health District Internal review application form or send us a letter putting forward your reasons why the decision should be changed.
A $40 application fee is payable via cheque / money order or by credit card using our online payment portal. If payment is made using the payment portal you can upload your Internal Review application form as an attachment or email to SLHD-GIPA@health.nsw.gov.au. If paying by cheque / money order please forward the Internal Review application form along with the $40 application fee via post to:
